The state of Minnesota requires a veterinarian referral. You will need to download this referral form above and either bring it into your vet’s office, or email the form to your vet’s office to be filled out. 

Once the form is filled out and signed have your veterinarian email the form to dramanda@balancedhead2tail.com. This email is a HIPPA secure email, so your information will be safe. If you have any questions please email or call me and I will be more than happy to help you with your questions.
